Who received what, in which size, and when to replace it. The Excel template is free.
In short: a PPE issue record tracks, for each wearer, the equipment issued: product reference, size, quantity, date of issue, replacement date and signature. French law does not require it under that name, but it shows that the employer supplies suitable PPE free of charge and replaces it, as the Labour Code requires. Other countries have their own rules.
Six articles of the French Labour Code (Code du travail), and what they mean for the record.

The employer also informs wearers of the risks covered and the conditions of use and provision (R4323-104), writes instructions for use (R4323-105) and trains wearers, with practice in wearing the equipment if needed (R4323-106).
Outside France: other countries have their own rules. In Great Britain, the Personal Protective Equipment at Work Regulations 1992 require suitable PPE, capable of fitting the wearer correctly. Since 6 April 2022, the 2022 amendment extends these duties to limb (b) workers, and HSE states that workers cannot be charged for the PPE they need. Check the rules that apply to you.
Record the size from the supplier's size chart for the product issued, not a "usual" size: an M is not the same from one supplier to another. Reference points: glove sizes, safety shoe sizes.

In France, no article of the Labour Code requires a document called a "fiche de dotation" (issue record). However, the employer must supply PPE free of charge, maintain and replace it (article R4323-95) and inform wearers (article R4323-104). A record signed on issue helps show this. Other countries have their own rules: check those that apply to you.
In France, no: PPE is supplied free of charge by the employer (article R4323-95) and health and safety measures must not entail any financial cost for workers (article L4122-2). In Great Britain, HSE guidance states that workers cannot be charged for the PPE they need to do their work.
The wearer and their employee ID, site and job role, then for each item: the PPE, the product reference, the size, the quantity, the date of issue, the planned replacement date and the wearer's signature.
Under the French Labour Code, PPE is for personal use. If the nature of the equipment and the circumstances require successive use by several people, the employer takes measures so that it causes no health or hygiene problem (article R4323-96).
The Personal Protective Equipment at Work (Amendment) Regulations 2022 came into force on 6 April 2022. They extended employers' and employees' PPE duties to limb (b) workers, typically people in casual or irregular work. They do not apply to the self-employed.
